On 04/15/2026 at 4:17PM, Licensing Program Analyst (LPA) Carol Fowler was at the facility for a complaint investigation (#15-AS-20260406090956), the following deficiencies were observed.
During the complaint investigation, LPA observed the following:
*Medication room unlocked which contained medication such as Benadryl.
The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties.
