Facility Evaluation Report
On 07/08/2025 at 10:30 AM, Licensing Program Analyst (LPA) David Doidge arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator Brittney Hall and explained the purpose of the visit. LPA explained to the Administrator that an inspection still needs to be completed even if there are no residents as long as the license is active.
During the visit, LPA did not observe any resident at the facility. No staff and resident interviews were conducted. LPA toured the facility including but not limited to residents’ rooms, bathrooms, kitchen, common area and backyard. LPA observed lighting in all rooms adequate for the comfort and safety of the residents. Hallway temperature was maintained at 70 degrees Fahrenheit. The hot water temperature in a shared bathroom was measured at 115 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. Sharps and toxins are locked and inaccessible to residents.
Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last purchased on 07/21/2025. Emergency Disaster Plan was last posted on 07/21/2025. First aid kit was observed to be complete. Emergency disaster and fire drill were last conducted on 07/21/2025.
There are no residents’ records. LPA reviewed one (1) staff record; which was complete.
