Facility Evaluation Report
On 05/16/2025 during the review of R1's file for the investigation of the death report, LPA's Observed a special incident report (SIR) dated 1/14/2025. The SIR stated that on 01/14/2025 at 9:30 AM, facility staff heard a loud noise, staff went into client's room and found him on the floor in the bathroom. When they lifted him up they noticed a bump on the left side of his head. Facility staff failed to seek medical attention for R1 at that time.
Deficiency was observed (see LIC 809D) and cited from the California Code of Regulations. Failure to correct deficiency by POC date may result in additional Civil Penalties.
Exit interview conducted. Appeal Rights and a copy of this report provided via email due to LPA not having a working printer.
