Facility Evaluation Report

On 05/16/2025 at 10 AM, Licensing Program Analysts (LPAs) Ardalan Gharachorloo and Greg Clark arrived unannounced to conduct a case management visit due to receiving a death report for R1 on 05/15//2025. LPAs met with De Mauriee Ingraham, Care Staff and explained the purpose of the visit. Administrator was not present during the visit.

During the visit, LPAs interviewed S1 and reviewed R1s file. S1 stated that when he arrived at work on 5/15/2025, he observed paramedic and fire department personnel at the facility. S1 later learned that R1 had apparently fainted in the bathroom and S2 was conducting CPR on R1. The paramedic fire department staff took over the CPR.

LPAs reviewed R1's record. R1 had lived at the facility for 25 years had history of seizures but was seizure free for at least 10 years and was not on any seizure medication. R1 was last seen by his primary physician on 05/10/2023 and no abnormalities were noted. R1 also had lab work done on 03/04/2025 and no abnormalities were noted.

No deficiencies cited during visit. Exit interview conducted and a copy of this report will be emailed to the administrator due to LPA not having a working printer.
