On 8/6/2025 at 9:30AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. LPA met with Administrator, "Ken" Nkemdirim Ugbaja and informed him the reason for visit.
During visit, LPA hand delivered the immediate exclusion letter for S1 to Administrator. LPA was informed that S1 haven't worked at the facility and was terminated in November 2024.
No deficiencies are being cited on this date.
