Facility Evaluation Report
On 3/13/2026 at 4:15 PM, Licensing Program Analyst (LPA) James Sampair arrived announced to conduct the Required Annual Inspection. Upon entry, LPA identified himself to Staff Member Joseph McCallum.
The facility has no clients currently. The Licensee will inform the LPA if she has clients at the facility again.
The LPA inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. The fire extinguisher was fully charged.
The carbon monoxide and smoke detectors were fully operational. The LPA observed required postings in the facility.
No citations were issued during the inspection.
Exit interview conducted and a copy of this report provided.
