Facility Evaluation Report
On 12/23/25 at 1:40 PM, Licensing Program Analyst (LPA) David Doidge conducted a case management visit. LPA met with Administrator Sarah Trail and explained the purpose of the visit.
LPA toured the facility including but not limited to bedrooms, bathrooms, dining room, living room and kitchen. LPA observed the facility is clean, odor free and in good repair. LPA also observed no clients live at the home.
Administrator Sarah Trail stated that effective 12/01/25, Regional Center of the East Bay (RCEB) has approved the facility to be operating under the Supported Living Services Program (SLS).
At 1:45 PM, Administrator Sarah Trail surrendered the facility's State License to LPA for facility closure.
No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
