Facility Evaluation Report
On 07/15/2024 at 10:15 AM, Licensing Program Analyst (LPA) David Doidge arrived unannounced to conduct the Required Annual Inspection. LPA met with Leslie Visbal Interim Executive Director and explained the purpose of the visit.
LPA toured the facility with Leslie Vibal and Operations Director Stefanee Hardaway, including but not limited to activity rooms, kitchen, bathrooms, office space, and the outside recreational area. Participants bring their own lunches and snacks. Emergency supplies, including water were observed. The hot water temperature in a shared bathroom was measured at 120 degrees Fahrenheit. Room temperature measured at 77 degrees Fahrenheit. Cleaning supplies are locked and inaccessible to participants. There were no bodies of water or fire safety hazards observed. Restrooms are maintained in safe and in sanitary operating condition, and additional equipment for the physically handicapped was observed. The facility is free of odors.
Emergency Disaster Plan was last posted on 05/15/2025. Emergency disaster and fire drill were last conducted on 07/15/2025. The carbon monoxide and smoke detectors were fully operational. First aid kit was observed to be complete.
LPAs reviewed five (5) residents records and five (5) staff records; all were complete.
