Facility Evaluation Report

On 5/12/2026 at 11:30 am, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual required inspection. LPA met with Administrator Michelle Bolivia and explained the purpose of the visit.

LPA toured the facility including but not limited to activity rooms, bathrooms, kitchen, and common area. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 112.7 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition.

Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 3/21/2026. The First aid kit was observed to be complete. LPA reviewed six (6) staff records and six (6) client records. During record review LPA observed that The Emergency Disaster Plan was last reviewed on 5/11/2025. The emergency fire drills were last conducted on 5/6/2026.
