Facility Evaluation Report
Licensing Program Analyst (LPA) Luisa Fontanilla arrived unannounced at around 10am and met with staff Jimmy Venoza. LPA explained to Venoza the purpose of the visit. The Administrator arrived at the facility at 10:18 am.
The facility reported that a bedridden resident on hospice was admitted to the facility on 5/11/2025. The facility does not have a bedridden fire clearance.
During the visit, LPA explained to the Administrator the difference between hospice waiver and fire clearance.
Deficiency is cited per Title 22 California Code of Regulations (refer to Lic 809D). Civil penalty of $500 is being assessed for today's visit.
Exit interview was conducted with the Administrator. Appeal Rights and a copy of this report were provided.
