Facility Evaluation Report

On 3/17/2026 at 3:00pm, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to conduct a Case Management visit. LPA met with Administrator, Julie Ann Caparino and explained the reason for the visit.

While LPA A. Gomez was conducting a complaint investigation 15-AS-20260310120011 on 3/17/2026. LPA observed 2 pairs of unlocked scissors in the kitchen by the sink in a gray container as well as unlocked over the counter medications in the cabinet in the living room.

The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
