Facility Evaluation Report
On 05/12/22 at 1:20PM while at the facility for another reason, LPA D Panlilio spoke with administrator (ADM) who stated that he did not file an incident report that happened at the facility on 04/24/22 involving client (C1) being repositioned incorrectly in his wheelchair by staff (S1) which prompted visiting family member to confront the staff on how to care for her loved one.
Deficiency is cited per Title 22 California Code of Regulations and listed on LIC 9099D. Failure to submit proof of correction (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties .
