The inspector’s account

On 07/18/2025 at 3:00pm Licensing Program Analysts (LPAs) L. Alexander and K. Nguyen arrived unannounced to conduct a Case Management visit. LPAs met with Administrator, Leea Burns.

While LPAs L. Alexander and K. Nguyen was conducting a Pre-Licensing Change of Facility Type (CHOFT) on 07/18/2025 the following deficiencies were observed during the tour of facility and record review.

THE FOLLOWING DEFICIENCIES WERE OBSERVED DURING VISIT:

At 12:19pm unlocked Chemical in garage on table and in file cabinet, big bathroom, underneath bathroom cabinet of small bathroom, and on the kitchen counter (i.e., Clorox Multi-Surface Cleaner and Bleach, Clorox Mist Spray, Pine-Sol)

At 12:20pm unlocked medication in file cabinet (Robitussin, Milk of Magnesia, and prescription Chlorhexidine…), and prescription thickener on the kitchen table from previous resident.

At 12:23pm unlocked hammer tool in garage, drill tool in client dresser, nail, and pair of scissors located in kitchen on dish rack.
