Facility Evaluation Report
On this day at around 2pm, LPA Luisa Fontanilla arrived unannounced to conduct a case management - incident visit and met with Humphrey Kasweka. Administrator Leea Burns was informed about the visit and she authorized Kasweka to sign the report.
On 5/9/2025, LPA conducted a case management visit in connection with an incident when Client 1 (C1) fell and sustained four stitches on the forehead. On said visit, LPA interviewed staff and obtained records.
Based on review records and interviews conducted, Staff 1 (S1) who was assigned to supervise C1 did not have the appropriate training as to C1's fall prevention plan. Based on interview with staff, C1's condition has improved since the current fall prevention plan was initially implemented. However, there has been no reassessment or update observed in C1's record.
Deficiencies are cited per Title 22 California Code of Regulations (refer to Lic 809D).
Exit interview was conducted with Kasweka and Appeal Rights was provided.
