On 08/05/2025 at 4:00 PM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a Plan of Correction (POC) visit. LPA met with Administrator, Leea Burns and explained the purpose of the visit.
On 07/18/2025 LPAs L. Alexander and K. Nguyen conducted an Pre-Licensing visit in which deficiencies were cited. The Plan of Correction (POC) due dates was 07/25/2025 and 08/01/2025. LPA was unable to conduct a 10-day POC visit. LPA toured the outside back yards and all items were removed.
Exit interview conducted, deficiency clearance letters and a copy of this report was provided.
