Facility Evaluation Report

At around 2:15 pm, LPA Luisa Fontanilla arrived unannounced to conduct a case management - incident visit. LPA met with staff Karen Parramore. The Administrator, Leea Burns was informed about the visit. She authorized Parramore to sign the report.

Back up Administrator Humphrey Kasweka arrived at around 3:35 pm.

During the visit, LPA spoke with Burns regarding Client 1 (C1) fall incident resulting to client sustaining cut on the forehead. LPA met with C1. LPA observed C1's forehead has 4 staples. LPA also observed fall mats and alarm mat by C1's bed. Parramore states the staples will be removed next week.

LPA obtained a copy of C1's Supervision Plan, staff schedule and staff training. The Administrator will meet with the team to come up with additional fall prevention plans for C1.
