Facility Evaluation Report
Licensing Program Analysts (LPAs) Luisa Fontanilla and Patricia Manalo arrived unannounced to conduct an annual required inspection and met with staff . LPAs explained to the purpose of the visit. Administrator certificate is current.
During the visit, LPAs observed three clients and 2 staff at the facility. LPAs inspected the facility inside and out including but not limited to 3 client rooms and 2 bathrooms. There was sufficient supply of perishable and non perishable foods. LPA observed a fire extinguisher that appeared full and was last serviced on 06/25/2024. There are no bodies of water observed. Hot water in the kitchen and bathroom measured at 109 degrees. Fahrenheit.
At around 2:36PM, LPAs reviewed 3 client files and 3 staff files. All records were observed complete.
The following deficiencies were observed:
Lysol cleaning chemicals were observed unlocked in the small bathroom at around 2:45 PM. Repeated Violation of $250 is assessed today.
Emergency disaster drills were observed not completed quarterly at around 4:30 PM
Deficiencies are cited per Title 22 California Code of Regulations (refer to Lic 809D).
Exit interview was conducted and Appeal Rights was provided.
