Facility Evaluation Report
On 07/20/2026 at 12:15PM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct a Proof Of Correction (POC) visit. LPA met with administrator Imelda DeLeon and explained the purpose of the visit. The facility currently houses three (3) residents.
The following deficiencies cited during the annual visit on 06/30/2026 were cleared during this visit:
80026(h) - LPA conducted a review of the residents' P&I Log for two residents (R1, R2). Both residents' logs were accurate and all residents' monies were accounted for.
No deficiencies cited during visit. Exit interview conducted. A copy of this report was made available to the administrator.
