On this day while at the facility conducting investigation related to complaint #15-AS-20240103160006, LPA observed one of the bathrooms has a lot of mold on the glass sliding door. LPA also observed that the floor covered with a wet rug appeared shaky and unsteady.
Deficiency is cited per Title 22 California Code of Regulations (refer to Lic 809D).
Exit interview was conducted and Appeal Rights was provided.
