Licensing Program Analyst (LPA) Luisa Fontanilla arrived unannounced to conduct a case management visit and met with staff Anita Tampos. LPA explained to Tampos the purpose of the visit. The Administrator arrived at the facility at 12:50 pm.
During the visit, LPA discussed with the Administrator that the following records need to be corrected/updated and send to CCL by 5pm on 5/23/2025:
