Facility Evaluation Report

On this day at around 3pm, LPA conducted a case management visit as follow up on the facility's informal meeting held on 2/19/2024. LPA was met by staff Menchie Matalang. The Administrator was not available but authorized Matalang to sign the report.

During the visit, LPA observed the facility has sufficient supply of perishable foods and non perishable foods.

There were plenty of milk, eggs, bread, meat, juice, fruits and vegetables observed. Non perishable foods were also observed sufficient.

There is no deficiency issued for today's visit.
