On this day at around 10:25 am, LPA Luisa Fontanilla arrived unannounced to conduct a case management visit to follow up deficiencies noted during the Regional Center of the East Bay (RCEB) QA visit conducted on 10/30/25. LPA met with Jasmine Williams and explained the purpose of the visit.
Deficiencies are cited per Title 22 California Code of Regulations (see Lic 809D).
