Facility Evaluation Report

On 12/5/2025 at 10:00AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a Required - 1 Year inspection. LPA met with Facility Manager, Tyrone Titus and explained the purpose of the visit. Program Director, Ken Manu was unable to be at the facility and designated Facility Manger to sign licensing reports.

LPA toured the facility including but not limited to bedrooms, bathrooms, dining area, kitchen, detached garage, and outdoor area. Smoke and carbon monoxide detectors were observed. Fire extinguishers were observed to be full and last serviced on 11/29/2025. One week of nonperishable and 2-day of perishable food supplies were available. There were adequate lights in each room. First Aid kit is complete. Last fire drill was conducted on 12/1/2025. LPA reviewed 6 clients and 5 staff files starting at 10:50AM. LPA reviewed a sample of client's medications during inspection.

At 10:30AM, LPA observed S4 does not have criminal record clearance after reviewing Guardian system. It was confirmed that S4 has been working at the facility.

At 1:15PM, LPA observed S5 does not have current First Aid training completed.
