Facility Evaluation Report

On 8/27/2025 at 9:10AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. LPA rang the door bell and knocked on the door without a response. LPA observed the facility van was not on the driveway. LPA observed sister facility's van was still present and drove to sister facility.

LPA returned to the facility at 10:45AM and met with Vice President of Operations, Alex Archangel.

During visit, LPA hand delivered the immediate exclusion letter for S1 to Vice President of Operations. LPA was informed that S1 works at G.L.O.M. ARF on Tuesdays through Saturdays from 12:00AM to 8:00AM. LPA obtained S1's contact information during inspection. LPA requested Licensee's contact information.

No deficiencies are being cited on this date.
