The inspector’s account

On 8/27/2025 at 9:30AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a case management visit to deliver an immediate exclusion letter. LPA met with caregiver, Willie Ferrell. Vice President of Operations, Alex Archangel arrived 30 minutes later.

During visit, LPA hand delivered the immediate exclusion letter for S1 to Vice President of Operations. LPA was informed that S1 works at G.L.O.M. ARF for overnight shift, but not at G.L.O.M. ARF 2. LPA requested S1's contact information and Licensee's contact information.

No deficiencies are being cited on this date.
