The inspector’s account

On 7/28/2022 at 8:45AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct an Infection Control Inspection. LPA met with nurse, Fe Herradura. Administrator, Marilyn Daguio arrived 30 minutes later.

Upon entry, LPA's temperature was checked and COVID-19 questionnaire was asked. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, living room, garage, and outdoor areas. LPA observed sign & symptoms, cough etiquette, and social distancing were posted in the common areas. Hand washing posters were posted at bathrooms and sinks.

During record review, LPA observed visitors log and temperature logs for clients. LPA observed facility has a copy of Mitigation Plan on file. Facility have documented staff's fit testing for N95 respirators. LPA observed PPEs, food, and paper supplies are sufficient.

At 9:00AM, LPA observed unlocked cleaning supplies and unlocked knives drawer in the kitchen. Staff lock up the cleaning supplies and knives drawer during inspection.

At 9:05AM, LPA observed unlocked medication in the small refrigerator and lockbox was broken. Staff locked up the medication and purchased a new lockbox during inspection.

At 9:15AM, LPA observed flooring between client's bathroom and hallway is in disrepair with black tapes used as transition material between the tile and hardwood flooring. The door frame guard is cracked for room 3.

The deficiencies were observed (see LIC 809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
