The inspector’s account

On 2/20/26, LPA K. Nguyen arrived unannounced to conduct a 10-day initial complaint. Upon arrival at around 9:15 am, LPA rang the doorbell and waited. LPA tried calling the facility phone, but was not able to leave messages because the mailbox is full. LPA walked to the back and saw a sign indicating that S1 is out of the office to call a number that was left on inside the window. LPA called no one answered. LPA walked back to the front, and a client had heard LPA. LPA introduce self to the client and asked if there were staff in the facility. Client responded “NO”. LPA confirmed with three other clients that there are no staff in the facility. LPA observed that multiple clients were left unsupervised in the facility. Staff later arrived at 10:30 am.
