Facility Evaluation Report
On 9/25/2025 at 1:30PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a Required - 1 Year inspection. LPA met with staff, Melodina Austria and explained the purpose of the visit. Administrator, Maria Janice Vizcarra arrived 30 minutes later.
LPA toured the facility including but not limited to bedrooms, bathrooms, dining area, kitchen, garage, and outdoor area. Smoke and carbon monoxide detectors were observed. Fire extinguisher was observed to be full and last serviced on 3/2/2025. Facility has two days of perishable and one week non-perishable food supplies available. Hot water temperature was measured at 105.2 degrees F in the kitchen sink. LPA observed grab bars and non-skid mat in the bathrooms. First Aid kit is complete. Last fire drill was conducted on 7/4/2025. Facility has a written emergency disaster plan.
LPA reviewed 5 residents files and 3 staff files starting at 1:45PM. LPA reviewed residents' P&I money with log. Facility has a current surety bond. LPA reviewed a sample of resident's medication during inspection.
No deficiencies are being cited on this date.
