Facility Evaluation Report
On 03/13/2026 at 8:45 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct 1-a Year Annual Required inspection. LPA met with Program Supervisor Wanda Willis and explained the purpose of the visit. Director Della Boughton was unavailable during the visit. The day program operates from Monday to Friday, 8:30 AM to 3:00 PM.
During the visit, LPA inspected the Adult Day Program (ADP), including but not limited to the bathroom, two activity rooms, reading room, resting room, etc. Hot water temperature measured at 109.7°F in the bathroom. Fire extinguishers observed and last serviced on 01/20/20/2026. The room temperature was observed at 70 degrees Fahrenheit. An emergency disaster drill was conducted on 1/23/26 (every six months).
LPA reviewed 6 staff and 5 client files. All staff are fingerprint cleared and associated with the ADP.
- At 11 am, LPA conducted a file review. Staff 1 (S1) and Staff 2 (S2) do not have TB in their files.
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
