The inspector’s account

On this day, March 12, 2026, at 10:55 am, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection and met with staff, Leticia Iroy, and informed the reason for visit. LPA spoke over the phone with Regina Luyon, administrator (ADM) who gave permission to Leticia Iroy to be with LPA during inspection. Jina Sunien, staff, arrived at 11:00 am. ADM arrived at 11:21 am.

LPA toured the facility inside out. LPA inspected the living room, dining area, kitchen, bathroom, residents rooms, front, side and backyard. Food supplies were checked and observed good for 2 days of perishables and 7 days of non-perishables. Central storage for medications and cabinets for cleaning supplies and sharps were observed locked.

Facility has smoke and carbon monoxide detectors that were tested and observed in operating condition on this day. Hot water temperature in the bathroom was tested and measured at 112.6 degrees Fahrenheit. Facility conducts disaster drills at least quarterly with fire and earthquake drills last conducted February 7, 2026 and January 20, 2026 respectively . Fire extinguisher checked, observed fully charge with receipt showed purchased on October 2, 2025.

LPA reviewed 5 staff and 4 residents records. Medications checked, and compared with LIC622 Centrally Stored Medication and Destruction Records and doctor's orders. Residents' P&I checked and compared with last recorded balance.
