The inspector’s account

On 6/4/26 at around 11:30 am, Licensing Program Analyst (LPA) Kelly Nguyen arrived at the facility to conduct an annual required inspection. LPA observed that there’s no one at the facility. LPA contacted Administrator (ADM) Angela Pascual to explain the purpose of the visit and that the phone number provided on FAS the voicemail was full, so LPA was unable to leave any messages. LPA waited outside the facility until 1:30 PM. Administrator arrived at 1:35pm.

LPA started the inspection with ADM and continued with ADM. LPA inspected the facility inside and out. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, and the front and backyards. Food supplies were observed to be good for 2 days of perishables and 7 days of non-perishables. Central storage for medications was observed to be locked.

The facility has 2-in-1 smoke and carbon monoxide detectors that were tested and observed in operating conditions on this day. The hot water temperature in one of the bathrooms was tested and measured at 110 degrees F.
