Facility Evaluation Report

On 04/06/2026 at 8:30 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct a 1-Year Annual Required inspection. LPA met with Administrator Joaquin Yambao and explained the purpose of the visit. The administrator's certificate is current, #70257135, valid from 2/9/25 to 2/8/27. The facility’s fire clearance was approved for four (4) ambulatory clients only.

LPA toured the facility, including both inside and outside, covering bedrooms, bathrooms, the kitchen, common areas, and the backyard. The facility has 2 bedrooms, all occupied by clients, and 2 bathrooms. There are no bodies of water on the premises. A comfortable temperature of 71 degrees Fahrenheit is maintained for the clients. LPA observed that the lighting in all rooms is adequate for their comfort and safety. The hot water in the shared clients’ bathroom was measured at 117.6 degrees Fahrenheit. All toilets, handwashing, and bathing facilities are safe, sanitary, and in proper working condition. An adequate supply of extra hygiene products was available for clients.
