On 11/19/2025 at 9:00 AM Licensing Program Analysts (LPAs) L. Alexander and K. Nguyen arrived unannounced to conduct a Case Management visit. LPAs met with Caregiver, Florante "Dan" Landingin. Dan phoned Licensee/Administrator, Victor De Leon to inform. Victor arrived shortly after.
LPA's conducted a record review on P&I monies and all monies balanced for two (2) of the four (4) clients. LPA's requested copies of bank receipts of P&I deposits for November 2023 thru February 2024.
No deficiencies cited during this visit. Exit interview conducted and a copy of this report provided.
