Facility Evaluation Report

On 5/05/25 at 8:45 AM, Licensing Program Analyst (LPA) K. Nguyen arrived and announced to conduct a one-year required annual inspection. LPA met with Administrator (ADM) Elena Rivera and explained the purpose of the visit. The facility currently has no residents/clients.

LPA toured the facility, including, but not limited to, bedrooms, bathrooms, the kitchen, common areas, and the backyard. Bedrooms and living rooms were furnished appropriately. Bathrooms were equipped with grab bars and non-skid mats. Lines and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout the facility. Room temperature was maintained at 68 degrees F. The hot water temperature at the kitchen sink was measured at 114 degrees F. The first-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. The fire extinguisher was last serviced on 2/19/2026. Fire Drill was last conducted on the week of March 3, 2026.
