Facility Evaluation Report
On 11/14/2025 at 12:45 PM, Licensing Program Analyst (LPA) P.Manalo arrived unannounced to conduct an annual required inspection. LPA contacted the Administrator/Licensee, Yuka Ono, and explained the purpose of the visit. Direct Care Staff, Lilibeth Toco, arrived shortly after. The facility is currently not in operation and has no clients or staff.
During the visit, LPA inspected the facility inside and out including but not limited to bedrooms, bathrooms, kitchen, common areas, etc. Hot water measured at 124.3 degrees. Fahrenheit. The facility is observed to be clean and odor free.
LPA observed carbon monoxide and smoke detectors.
There are no staff or client files for review.
Exit interview conducted and a copy of this report provided.
