Facility Evaluation Report

On this day, August 15, 2025, at 1:40 pm., Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with staff, Alyssa Reinee Maralit and John Jugarap. LPA called and spoke over the phone with Simporiana 'Bing' Jugarap, administrator (ADM), who authorized Alyssa Reinee Maralit to be with LPA during inspection. ADM arrived around 2:22 pm. LPA also met with other staff, Josejoy Penaranda and Lolita Gevana.

LPA toured the facility inside out. LPA inspected the kitchen, dining area, living room, bedrooms, bathroom, garage, side yard and backyard. Food supplies were observed good for 2 days of perishables and 7 days of non-perishables. Central storage for medications and drawers where sharps are kept were observed locked.

Fire extinguisher was observed fully charge with tag showed serviced February 14, 2025. Carbon monoxide and smoke detectors were tested and observed in operating condition. Hot water temperature in the common bathroom was tested, and measured at 105 degrees Fahrenheit. Facility conducts disaster drills every month and records showed last conducted August 2, 2025.
