Facility Evaluation Report
On this day, 08/07/2026, at 3:25 pm, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with staff, Kenneth Lal. LPA called and spoke over the phone with Joseph Crisol, administrator (ADM), and informed the reason for visit. ADM stated he cannot come to the facility and gave permission to Kenneth Lal to be with LPA during inspection and to sign and receive this report.
LPA toured the facility inside out. LPA inspected the dining room, kitchen, receiving area, bathrooms/shower rooms, front yard, side yard and backyard. LPA randomly selected 6 bedrooms for inspection. Food supplies were observed good for 2 days of perishables and 7 days of non-perishables. Central storage for medications was observed locked.
Facility has carbon monoxide and smoke detectors that were tested and observed operational on this day. Hot water temperature in one of the bathrooms was tested and measured at 110 degrees Fahrenheit. Fire extinguishers checked, observed fully charge with tags showed serviced 03/17/2026. Facility conducts disaster drills every quarter and records showed last conducted 05/05/2026.
