Facility Evaluation Report

While at the facility for other reason, Licensing Program Analyst (LPA) Delmundo learned that resident (R1) who was admitted to the facility on July 2025 left the facility unnoticed on the day R1 was admitted. LPA conducted interviews, reviewed R1's record and obtained copies of documents. LIC602 Physician's Report showed R1 can leave the facility unassisted, however, administrator did not submit an incident report.

Deficiency is cited from Title 22 California Code of Regulations and listed on 809D. Failure to submit proof of correction may result and any repeat violation within 12 month period may result in civil penalty.

Deficiency and plan and proof of correction were discussed with Joseph Crisol, administrator.

Exit interview conducted. Appeal Rights, LIC9098 Proof of Correction form and copy of this report provided.
