Facility Evaluation Report

On this day, 1/06/26, at 10:40 am, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct a case management visit as a result of the Department receiving information that resident (R1) moved-back to the facility. LPA met with George Vidican, licensee-administrator, and informed the reason for visit.

On 1/05/26, Licensing Program Manager (LPM) Fong and LPA Delmundo had a call conference with licensee, and confirmed that R1 was moved-back by R1's sister in December 2025. Licensee confirmed he can no longer provide R1's care needs and that he does not have caregiver nor be able to get a caregiver. Currently, the facility does not have other residents except R1 whose care is provided by R1's sister (FM), but FM is not a facility staff.

Deficiency is cited from Title 22 California Code of Regulations and listed on 809D. Failure to submit proof of correction by plan of correction due date may result in civil penalty.

Deficiency and plan and proof of correction were discussed with licensee.
