Facility Evaluation Report

On 7/7/2026 at 9:00 am, Licensing Program Analyst (LPA) K. Nguyen conducted an unannounced annual required inspection. LPA met with DSP staff Gerard Yabut. Gerard informed Administrator (ADM) Corshema Taylor via telephone, and LPA explained the purpose of the visit. ADM arrived at the facility at around 10:00 a.m.

The facility is approved for four (4) clients, all of whom may be ambulatory. LPA toured the facility, including but not limited to bedrooms, bathrooms, kitchen, common area, and backyard. The facility consists of three (3) bedrooms, all of which are occupied by clients, and two (2) shared client restrooms. All indoor and outdoor passageways are kept free of obstructions. There are no bodies of water. A comfortable temperature for clients is maintained at 71 degrees Fahrenheit.
