Facility Evaluation Report
On 7/24/2025 at 1:00 pm, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual required inspection. LPA met with DSP staff Yolanda Mendoza. Yolanda informed Administrator (AD) Corshema Taylor via telephone and LPA explained the purpose of the visit. AD arrived to the facility at around 2:00 pm.
The administrator currently holds a certificate (#7011569735) that expires on 09/15/2026. The facility is approved for four (4) clients all may be ambulatory. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of three (3) total bedrooms which all bedrooms are occupied by the clients and two (2) shared client restrooms. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 71 degrees Fahrenheit.
LPA observed lighting in all rooms are adequate for the comfort and safety of the clients . Hot water temperature in the shared clients’ bathroom was measured at 108.4 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. There is a minimum of 7-day supply of non-perishables and 2-day perishables food supply. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 12/09/2024. The Emergency Disaster plan was observed to be last updated on 5/28/2025 and the fire drills were observed to be last conducted on 7/16/2025. First aid kit was observed to be complete.
