Facility Evaluation Report
At 12:55 pm on this day, April 15, 2025, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with staff, Caridad Escarez, and informed the reason for visit. LPA called and left message on Bonnie Jean Tabing's, administrator (ADM) voicemail. ADM arrived with other staff, Timothy Joseph Pascual, at around 1:17 pm.
LPA started the inspection with Caridad Escarez and continued with ADM. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms. side yard and backyard. Food supplies were observed good for 2 days of perishables and 7 days of non-perishables. Central storage for medications and storage for cleaning supplies were observed locked.
On this same day, LPA obtained copy of building permit for the Accessory Dwelling Unit (ADU) added at the property behind the facility.
Facility has carbon monoxide and smoke detectors that were tested, and observed in operating condition on this day. Facility conducts disaster drills at least every two months and records showed last conducted March 21, 2026. Fire extinguishers checked, observed fully charge with tags showed serviced January 20, 2026. Hot water temperature was tested and measured at 106.8 degrees Fahrenheit.
LPA reviewed 5 staff and 5 residents records. Medications were checked and compared with doctor's order and LIC622 Centrally Stored Medication and Destruction Record. P&I were checked and compared with last recorded balance.
