Facility Evaluation Report

At 1:00 p.m., on this day, April 29, 2026, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. No one was at the facility when LPA arrived. LPA called and spoke over the phone with Adaobi Ezenekwe, administrator(ADM), and informed the reason for visit. ADM arrived at 1:31 pm.

LPA toured the facility inside out with ADM. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, front, side and backyard.

Facility has carbon monoxide and smoke detectors that were tested and observed in operating condition. Fire extinguisher checked, observed fully charge with tag showed serviced March 19, 2026. Hot water temperature in the common bathroom was tested and measured at 110 degrees Fahrenheit.

LPA reviewed 1 staff record. Currently, facility does not have resident.

LPA discussed the payment of annual fee. On this same day, ADM paid the annual fee along with the late fee charge and provided to LPA copy of the proof of payment.
