Facility Evaluation Report
On 05/28/2026 at 8:00 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct a 1-Year Annual Required inspection. LPA met with Licensee/Administrator (ADM), Ricky Ng, and explained the purpose of the visit. The facility’s fire clearance was approved for a capacity of four (4). In which two (2) can be non-ambulatory and the other two (2) can be Ambulatory. Administrator Certificate is active and current.
LPA toured the facility, including but not limited to bedrooms, bathrooms, kitchen, common area, and backyard. The facility consists of 4 bedrooms, of which 3 are occupied by clients and 1 by staff. All indoor and outdoor passageways are kept free of obstructions. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed that the lighting in all rooms is adequate for clients' comfort and safety. The hot water temperature in the shared clients’ bathroom was measured at 110.1 degrees Fahrenheit. All toilets, handwashing, and bathing facilities are safe, sanitary, and in good working order. Extra hygiene products were available to clients.
Smoke detectors and carbon monoxide detectors were in operation during the visit. The fire extinguisher was last purchased on 1/13/2026. Emergency Disaster Drill was last posted on 01/31/2026. The first aid kit was observed to be complete. The fire drill was last conducted on 04/18/2026. Centrally stored medication and sharps were locked and inaccessible to residents. There is a minimum of one week's supply of nonperishable and 2 days' supply of perishable foods.
At 9:30 am, LPA reviewed the records of 4 residents. At 10:30 am, LPA reviewed 4 staff records, and 4 of 4 have current first aid training and are associated with the facility. LPA sample clients' medication.
No deficiencies cited during visit. An exit interview was conducted, and a copy of this report was provided.
