Facility Evaluation Report
On 08/6/2026 at 8:00 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct a 1-Year Annual Required inspection. LPA was greeted by care staff Terrence Daanton and explained the purpose of the visit. The Administrator (ADM), Ernesto Golez Jr, arrived at a later time. Administrator certificate is current. The facility’s fire clearance was approved for four (4) non-ambulatory clients.
LPA toured the facility, including but not limited to bedrooms, bathrooms, kitchen, common area, and backyard. The facility consists of 3 total bedrooms, all of which are occupied by the clients. All outdoor and indoor passageways are kept free of obstruction. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed that lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 111.04 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary, and in operating condition. There is a minimum of one week's supply of nonperishable food and a 2-day supply of perishable food.
Smoke detectors and carbon monoxide detectors were in operation during the visit. The fire extinguisher was last serviced on 4/21/2026. Emergency Disaster Drill was last posted on 02/05/2026. Fire Drill was conducted on 7/25/26. First aid kit was observed to be complete.
At 9:30 AM, 4 of the clients’ records were reviewed. At 10:00 AM, 3 staff records were reviewed, and 3 of 3 have current first aid training and are associated with the facility. At 10:30 AM, LPA reviewed the clients’ medications. At 10:50 AM, LPA reviewed clients’ P&I money.
