The inspector’s account

On 08/26/2025 at 9:00 AM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Rudy Golilao, and explained the purpose of the visit. Administrator certificate is current. The facility’s fire clearance was approved for four (4) non-ambulatory clients.

LPA toured the facility including inside and out but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 3 total bedrooms which all bedrooms are occupied by the clients. All outdoor and indoor passageways are kept free of obstruction. A comfortable temperature for clients is maintained at 74 degree Fahrenheit. LPA observed lighting in all rooms is adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 109.4 degree Fahrenheit. All toilets, hand washing and bathing are safe, sanitary and in operating condition. There is a minimum of one week supply of nonperishable and a 2-day perishables food supply.

Smoke detectors and carbon monoxide were in operation during the visit. Fire extinguisher was last serviced on 04/29/2025. Emergency Disaster Drill was last posted on 07/31/2025. First aid kit was observed to be complete.

At 9:36 AM, 3 of the clients’ records were reviewed. At 9:54 AM, 3 staff records were reviewed and 3 of 3 have current first aid training and associated with the facility. At 10:15 AM, LPA reviewed all the clients’ medications. The facility does not handle clients’ P&I money.

No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
