The inspector’s account

On this day at 11:20 am, March 27, 2025, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with staff, Jose 'Jay' Fernandez and

Milagros 'Mel' Fernandez, and informed the reason for visit. LPA called and spoke over the phone with Victoria "Vickie' Canovas, administrator (ADM), who authorized the staff to be with LPA in touring the facility. ADM arrived at around 12:05 pm.

LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, sun/recreation room, front, side and backyard. Food supplies were observed good for 2 days of perishables and 7 days of non-perishables. Central storage for medications and storage for cleaning supplies were observed locked.

Facility has carbon monoxide and smoke detectors that were tested and observed in operating condition. Facility conducts fire and earthquake drills at least every quarter, and records showed fire drill last conducted 1/04/25 and earthquake drill 1/06/25. Hot water temperature in the one of the bathrooms was tested and measured at 120 degrees Fahrenheit. First aid kit was observed complete with manual.

LPA reviewed 4 residents and 3 staff files. Residents medications were checked and compared with doctor's orders and LIC622 Centrally Stored Medication and Destruction Records. Residents' P&I were checked and compared with last recorded balance.
