Facility Evaluation Report

Licensing Program Analyst (LPA) Delmundo arrived to the facility unannounced for a complaint (15-AS-20220321081012) and met with Administrator Kolu Sokodolo and Regional Director Lorenzo Pennix Upon entry, LPA observed the smoke detectors chirping. Kolu indicated that they replaced the battery and it still chirp. LPA verified and both Kolu and Lorenzo said the unit has been chirping since Friday, March 25, 2022. They haven't had it checked nor have hired a contractor to check the unit.

Deficiency is cited from Title 22 California Code of Regulations (see 809D). Failure to submit proof of correction by plan of correction due date may result in civil penalty.

Deficiency and plan and proof of correction were discussed.
