Facility Evaluation Report
On 6/30/2025, at 8:45 AM, Licensing Program Analyst (LPA) James Sampair arrived announced to conduct this Required 1 Year inspection. Upon entry, the LPA confirmed the purpose of the visit with Assistant Administrator Ramon Romo .
Because there are currently no clients or staff members, and there have not been any for more than 2 years, there is no documentation for administrators, staff, or clients. Additionally, there is no food or required postings.
The LPA did inspect the physical plant inside and outside of the facility, which included the kitchen, dining room, common areas, bedrooms, storage, and the back yard. The physical plant did include adequate appliances for food storage and preparation, and there was locked storage for the central storage for medications and cleaning supplies or dangerous objects to keep them inaccessible to clients. The facility has working smoke and carbon monoxide detectors. The fire extinguishers were serviced on 5/8/2025.
Before any clients can be admitted, the appropriate number of staff members, food, and required postings must be displayed. Additionally, the required documentation for administrators, staff, and clients must be produced and stored on site at the facility.
No citations were issued during the inspection.
