The inspector’s account

On 5/20/ 2025, at around 11:30 am, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct an annual required inspection. LPA met with Administrator Lilibeth 'Beth' Ordiniza and explained the purpose of the visit. LPA observed that there are no clients at the facility. All clients went to the day program.

LPA toured the facility inside and out with Beth. LPA inspected the kitchen, dining areas, living room, and family rooms. Bedrooms, bathroom, side yard, and backyard. Food supplies were observed to be good for 2 days of perishables and 7 days of non-perishables. Central storage for medications and storage for cleaning supplies were observed to be locked.

The facility has carbon monoxide and smoke detectors tested and observed to be functional. The facility conducts earthquake and fire drills at least every quarter, and records showed the last conducted in May 2025. Fire extinguisher checked, observed fully charged with tag showing serviced June 4, 2024. Hot water temperature measured at 116.2 degrees Fahrenheit. Facility liability insurance expired on 3/22/2026. The facility vehicle registration and insurance are current.

LPA reviewed 5 staff and 5 clients'' records and interviewed 2 staff. Medications were checked and compared with the doctor's order and the LIC622 Centrally Stored Medication and Destruction Record. P&I were checked against records.

No deficiencies cited during visit. An exit interview was conducted, and a copy of this report was provided.
